Ohio law requires that ALL students entering grade 12 MUST have 2 doses of the Meningococcal (MCV4) vaccine administerd prior to entry. If the first dose of MCV4 was given on or after their 16th birthday, a second dose is not required.
Please be sure this vaccination(s) has been given to your child prior to the start of the school year. Please provide the school documentation within the first week of school, from your child's physician or health department, verifying that the vacination(s) are complete.
